Why do I gain weight in midlife even though nothing changed?

Several small things at once. Lean mass declines gradually, so resting energy use falls a little; daily movement usually falls more than anyone notices; and for women the menopause transition roughly doubles the rate of fat gain. Individually each is small, and together they produce a slow drift.

"My metabolism slowed down" is the usual explanation and it is doing a lot of work for one phrase. Unpacking it is worthwhile, because some of what is inside responds to what you do and some does not.

The first row is the one worth dwelling on. Muscle is metabolically active tissue, so losing it reduces resting energy use directly. That means part of what gets called a slowing metabolism is a change in what the body is made of rather than a change in how it runs, and that part has a lever attached.

The arithmetic of a slow drift

Half a kilogram a year is invisible. It is roughly 10 calories a day of imbalance, which is less than a mouthful.

Over a decade it is 5 kg, arriving without any month in which anything noticeably changed. This is why the experience is so consistently "I have not done anything differently": at that scale, nobody has.

It also explains why the fix is not dramatic either. A few hundred calories a day, some resistance training and a protein target are enough to reverse a drift that took years to accumulate.

The menopause part, specifically

For women there is a distinct and measurable change on top of the general age effect. In a cohort followed by DXA through the transition, the rate of fat gain roughly doubled at its start, while lean mass began to fall.1

Those two moving in opposite directions is why body weight is such a poor guide through this period, and that measurement problem has its own page.

The hormonal change itself is not something diet reverses. The lean mass side is, which is where the effort is worth putting.

What responds

  • Protein toward the older-adult range. At least 1.0 to 1.2 g per kilogram a day, which for most women in this group is somewhere between 65 and 95 g.2
  • Resistance training, two or three sessions a week. This is the half that gives lean mass a reason to stay, and starting after 40 works.
  • Daily movement, which usually falls more between 40 and 55 than anybody realises. Steps, stairs, standing, walking to things.
  • A week of actually measuring intake, once, if the drift has been going on for years. Not forever, and long enough to see whether portions moved.

If you do decide to lose some of it, do it in a way that protects the lean mass, because the composition of the loss matters more here than at 25: higher protein groups in the trials kept about 0.43 kg more fat-free mass than standard protein groups over roughly twelve weeks.3 The three levers are the same as ever.

Weight that changed suddenly rather than drifting, or that came with other symptoms, is a reason to see a doctor rather than to adjust a diet. Less abruptly than the phrase suggests. A meaningful part of what changes is body composition rather than metabolic rate itself: losing lean tissue reduces resting energy use, because muscle costs something to maintain. That is a change you can act on, which is more useful than treating it as an age effect.

Why is it happening when I have not changed anything?

Because several small things changed without being decisions. Slightly less muscle, slightly less movement through the day, slightly larger portions, slightly less sleep. None of them are noticeable and the sum is a few hundred calories a day, which is exactly the size of a slow drift.

Can I stop it?

You can influence the lean mass part directly, through protein and resistance training, and the movement part directly by moving more. The hormonal change through the menopause transition is not something diet reverses. Acting on the first two is what is available, and it is more than nothing.
